New Patient Guide to Getting Started

Welcome to WiseMinds Center for Integrative Telepsychiatry, and thank you for choosing us as your behavioral health and wellness provider. Our primary goal is to provide quality patient-centered care that is easily accessible and responsive to you in your time of need. Our staff includes a team of professionals who will consistently strive to exceed your expectations to ensure that your experience with us is as comfortable as stress-free as possible.

Here you will get the important details on how to get started with services to include; completing your patient portal registration, submitting intake paperwork, and preparing for your scheduled appointment.

The New Patient Guide will be made available once you have initiated the completion of your intake forms and consents.

Kareo Portal Registration

Once your appointment is scheduled and confirmed by our office, you receive an emailed link to activate your patient portal. Here you will upload your driver’s license, the front/back of your insurance cards. If the patient is a minor, please upload a photo of the minor.

Intake Forms and Consents

We respect your time. In order for you (and the other patients on the schedule) to be seen with minimal delays, patient registration and paperwork must be completed BEFORE your appointment. Therefore, plan to have all forms completed by 5 pm (business days) the day before your scheduled appointment time.

Prepare for your appointment

All patients who have a confirmed appointment will get an email with a link for the telehealth platform. We send out appointment reminders 7-days, 4-days, and 90-minutes prior to your appointment. If you misplace the email with the telehealth link, please email us on the day of your appointment for another link. Please refer to our patient guide regarding our cancellation and no-show policy./p>

Please remember that all patients must be in the provider’s jurisdiction for services.
